Requirements
Must have
- LPN or RN license
- Graduate of an accredited nursing program
- Experience in psychiatric, mental and behavioral health services
- Willingness to travel throughout assigned service area
- CPR (course can be provided during training)
Nice to have
- Experience working in LTC facilities as well as behavioral health
What you'll actually do
- Address the mental, behavioral and physical health needs of clients weekly, biweekly or monthly
- Complete in-person nursing assessments and medication and chart reviews in LTC facilities
- Educate caregivers and clients on prescribed medications and monitor for possible side effects
- Coordinate community services and correspond with physicians, pharmacies and treatment team
- Assist clients in building coping skills and creating annual goals related to their diagnosis
- Help train and mentor peers
