Requirements

NJ LSW or LCSW
Graduate of an accredited school of social work (MSW)
Previous medical hospital social work experience
Excellent communication, negotiation and conflict resolution skills
Knowledge of trends and advances in clinical practice and healthcare informatics
Knowledge of state regulatory standards related to social work
Rapid cycle change or clinical performance improvement expertise
Availability for weekend rotation

Assess patients' psychosocial situation and plan interventions and treatment goals
Interview patients, family members and significant others to assess social work needs
Provide psychosocial support to patients and families during adjustment to illness
Coordinate psychosocial discharge planning and appropriate after-care services
Document psychosocial assessments, interventions and discharge planning per regulations
Develop a social service network with city, state and other governmental services
