Requirements
Must have
- Associate's degree minimum
- 1+ year of health homes, HARP or case management experience
- Strong working knowledge of local community resources
- Experience navigating data management systems, such as Salesforce
- Ability to work with data reporting, documentation and outcomes
- Strong communication and assessment skills
- Ability to manage high volume caseloads
- Ability to travel in the field to accompany patients to appointments
Nice to have
- Bachelor's degree
- Master's degree
- 3+ years of Health Homes experience
- 3+ years of HARP experience
- Spanish
What you'll actually do
- Screen for Health Home functional scale eligibility and conduct assessments and reassessments of patient needs
- Collaborate with medical providers and patients to develop and coordinate Health Homes compliant care plans
- Provide direct service to a caseload of chronically ill patients through referrals and local resources
- Coordinate patient services with pharmacies, insurance companies, hospital discharge planning and caregivers
- Provide crisis intervention when needed
- Conduct home visits and document all services in accordance with Health Homes standards
