Bachelor's degree with at least 12 college credits in sociology, social welfare, psychology or related field
Or 12 related college credits and 1+ year as County Social Services Aide 3 or similar case management role
Or 12 related college credits and 2 years' professional experience in developmental disabilities or related field

Obtain social information for development of treatment plans and enrollment into BH/IDS services
Assist consumers in filing applications for SSI, DPW and VA benefits
Participate in development of Individual Service Plans and monitor residential and day program providers
Maintain files, records and documentation for continuity of consumer care and act as consumer advocate
Coordinate and direct meetings to assess consumers' progress in charted areas
Provide crisis intervention and emergency services as coordinated by the supervisor
