Must have
- Junior level
- Current score on file with the State Civil Service Commission for this job title
- Bachelor's degree with 12+ college credits in sociology, social welfare, psychology or related social science
- Or 2 years as County Social Service Aide 3 plus 2 years of college coursework with 12 related credits
- Or 12 related credits plus 1 year as County Social Services Aide 3 or similar case management position
- Or 12 related credits plus 2 years' professional experience in developmental disabilities or related field
What you'll actually do
- Obtain social information for treatment plans and enrollment into BH/IDS services
- Assist consumers in filing applications for SSI, DPW and VA benefits
- Participate in developing Individual Service Plans and monitor residential and day program providers
- Maintain files, records and documentation and advocate for appropriate consumer treatment
- Coordinate and direct meetings to assess consumers' progress
- Provide crisis intervention and emergency services as coordinated by the supervisor
